BY CLICKING "I AGREE," CHECKING A BOX INDICATING ACCEPTANCE, OR OTHERWISE AFFIRMATIVELY ACCEPTING THIS CONSENT, YOU ACKNOWLEDGE THAT YOU HAVE READ, UNDERSTOOD, AND AGREED TO BE BOUND BY IT. IF YOU DO NOT AGREE, DO NOT CREATE AN ACCOUNT, COMPLETE AN INTAKE, OR USE THE SERVICE.
1. Purpose of this consent
The purpose of this Informed Consent for Telehealth Services ("Consent") is to give you information about telehealth and to obtain your informed consent to receive healthcare services by telehealth.
Care is delivered by licensed clinicians — which may include licensed providers, nurse practitioners, and licensed provider assistants ("Providers") — who practice through one or more independent professional entities ("Medical Groups"). Blissley is a technology and administrative platform operated by TheFactual LLC. Blissley does not practice medicine, does not employ Providers, and does not direct or interfere with their independent clinical judgment.
A clinician at any one of the following Medical Groups may be involved in your care:

The telehealth platform through which these Medical Groups deliver care is operated by OpenLoop Health, Inc., 317 6th Avenue, Suite 400, Des Moines, Iowa 50309. OpenLoop Health, Inc. is a third-party beneficiary of this Consent.
Which Medical Group treats you depends on the state you are located in and the service you request. You may confirm the name, credentials, and license of your treating clinician at any time — see Section 9.

2. What telehealth is
Telehealth is the delivery of healthcare using electronic communications between a patient and a clinician who are not in the same physical location. It may include:
- Asynchronous ("store-and-forward") review, in which you submit clinical information through an online intake and a Provider reviews it and responds without a real-time conversation;
- Synchronous consultation by live video or telephone, where clinically indicated or where required by the law of your state;
- Secure messaging with your care team through the patient portal;
- Electronic transmission of a prescription to a licensed pharmacy.
Most Blissley evaluations are asynchronous. You complete a clinical intake, a Provider reviews your submission, and you receive a decision without a live appointment.
Telehealth laws differ by state. Some states permit asynchronous evaluation; others require live video or telephone contact before a prescription may be issued. Where your state requires a synchronous consultation, we will contact you to arrange one before any prescription is written. If you cannot be reached, or a required consultation cannot be completed, no prescription will be issued and you will not be charged.
3. What you are consenting to
By accepting this Consent, you acknowledge and agree that:
- Your Provider will not be physically present with you, and no physical examination will be performed.
- You consent to evaluation, diagnosis, treatment, and prescribing by telehealth, including by asynchronous review where permitted in your state.
- You consent to your Provider accessing and importing your medical records and medication history as necessary to provide care.
- You will complete your intake and any consultation from a private location, and you understand your Provider will do the same.
- You will provide complete and accurate information. Incomplete or inaccurate information may lead to an incorrect assessment or an unsafe treatment decision.
- You are physically located in the United States, and in the state matching your shipping address, at the time of your evaluation.
- You are 18 years of age or older.
4. Benefits and risks
Potential benefits include access to care without travel, greater convenience and privacy, and shorter waits for evaluation.
Potential risks and limitations include:
- Information transmitted may be insufficient for sound clinical decision-making, and your Provider may be unable to reach a conclusion without an in-person examination or laboratory testing;
- Delays in evaluation or treatment caused by technology failures;
- In rare cases, transmission errors, data corruption, or unauthorized access to your protected health information despite reasonable safeguards;
- Telehealth may not be appropriate for your condition, in which case your Provider will decline to treat and will advise you to seek in-person care.
You may withdraw this Consent at any time by contacting support@blissley.com. Withdrawal does not affect care already provided.
5. No guarantee of treatment
Completing an intake and enrolling in a plan does not guarantee that you will receive a prescription.
A Provider will determine independently whether treatment is medically appropriate for you. If it is not, no prescription is issued, no medication is dispensed or shipped, your plan is cancelled, and you receive a full refund in accordance with our Refund & Cancellation Policy.
Your Provider's decision is based on your clinical information, medical history, and the applicable standard of care — not on which treatment you selected or expressed interest in.
6. Compounded medications
Some medications prescribed through Blissley are compounded, meaning they are prepared for an individual patient by a state-licensed 503A compounding pharmacy.
Compounded medications are not approved by the FDA. The FDA does not review compounded preparations for safety, effectiveness, or quality. Compounded medications are not generic equivalents of, interchangeable with, or approved substitutes for any FDA-approved product.
A compounded medication will be prescribed only where your Provider determines, in independent clinical judgment, that a compounded preparation is appropriate for you — for example, because a commercially available product is not clinically suitable for your specific needs. Price is not a factor in that determination.
You may ask your Provider about FDA-approved alternatives at any time.
7. Your right to in-person care
Telehealth is not a replacement for in-person medical care. You may refuse telehealth at any time and seek care in person without affecting your right to future services. We encourage you to maintain a relationship with a primary care clinician and to share your Blissley treatment information with them.
8. Privacy and medical records
Your protected health information is handled in accordance with HIPAA and our Privacy Policy. A medical record of your telehealth encounter is created and maintained by your Provider Group. You may request a copy of your records at any time.
Medical records for adult patients are retained for a minimum of ten (10) years from the date of the last encounter, or longer where state law requires.
Telehealth encounters are not recorded by video or audio without your separate written consent.
9. State-specific disclosures
Several states require that patients receive identifying and licensure information about the clinician treating them. If you are located in California, Massachusetts, Rhode Island, Vermont, Georgia, or any other state with a comparable requirement, you may obtain the name, credentials, license number, and licensing state of your treating Provider — before or after your evaluation — by:
- Messaging your care team in the patient portal;
- Emailing support@blissley.com; or
- Calling the number listed on our Terms & Conditions page.
This information is provided on request at any time, at no cost.
10. Financial responsibility
Blissley does not bill insurance. You are responsible for the cost of your plan as disclosed at checkout. Billing terms, renewal dates, and cancellation are described in our Terms & Conditions and Refund & Cancellation Policy.
